1. Scanned of certificate of the latest attended school (in English) or scanned of academic record from home university or the last attended school (in English)
2. Guarantor Financial Statement (form as attached on the application form)
3. Color scanned of valid passport (passport cover, id page), has 18 months validity for 6 months study, 24 months validity for 1 year study (EXAMPLE)
4. Declaration form (form as attached on the application form)
5. Recent Color Closed up photo should be in jpeg/jpg
- Formal Dress (EXAMPLE)
- In Color (EXAMPLE)
- Taken in front of a plain white or off-white background (EXAMPLE)
- Glasses are not acceptable (EXAMPLE)
- With a neutral facial expression and both eyes open (EXAMPLE)
- Taken in full-face view directly facing the camera (EXAMPLE)
6. Medical Statement (form as attached on the application form)
Additional documents for Exchange program
1. A cover letter from Home University to the Rector of Udayana University
2. Learning agreement
ELIGIBILITY: § The documents should be formed in PDF/Jpeg § No more than 300kb each document | NOTE FOR ONLINE CLASS: § The student doesn’t need to submit visa requirements § Study permit won’t be issued § Passport can be replaced by Student card/Id Card |
